Job Duties

Develops, implements, and supervises programs to help residents achieve maximum capabilities, and to prevent or minimize new or progressive disabilities. Plans are developed for 100% of identified residents. Provides clinical management of long term patients.

Evaluates, prescribes designs, fabricates, requisitions, and coordinates implementation and repair of adaptive equipment. Coordinates with vendors, purchasing, fiscal, living units, and other departments.

Works cooperatively with other IDT members to design integrated plans and provide supportive documentation. Goals, objectives, and plans are identified for all patients receiving PT services within established time frames or of recommended changes and all target dates are current. Ensures assess-ments are completed within established timeframes and that cost information is forwarded to the appropriate sources in a timely manner. Attends and actively participates in clinics and meetings. Uses the appropriate lines of communication and resources within the system to facilitate change and resolve problems.

Provides direction and training to new staff, student interns, families, volunteers, and communities in physical management and PT-related services. Documents observations of plan implementation to assure service delivery and quality. Documents training and remediation sessions on required facility forms for R & T. Assists with additional activities as assigned.

Attends department and other meetings. May conduct department meetings as assigned and completes ancillary documentation and communication. Follows all safety rules and regulations. Initiates and recommend appropriate corrective action necessary when violations are seen. Maintains clean and safe environments ensuring orderly work area throughout the unit are maintained. Ensures fall prevention devices/care plans are followed and interventions are in place.

All other duties assigned by the Director of Rehabilitation.

Minimum Qualifications

Knowledge of assessment and treatment in general and neurodevelopmental physical therapy, standards of practice at ICF/MRs, and effective resource management principles. Skilled in verbal and written communication, decision-making, problem-solving, and conflict resolution. Ability to lead and coordinate work of the physical therapy department, and cross-functional departments as needed.
