Job Duties
The Clinical Social Worker provides comprehensive, recovery-oriented clinical and case management services to individuals with serious mental illness and co-occurring disorders. This role integrates direct clinical care, discharge planning, interdisciplinary collaboration, and community coordination to support successful patient outcomes and continuity of care.
Minimum Qualifications
Master’s Degree in Social Work from an accredited educational institution.
Experience providing Social Work Services to clients diagnosed with SMI and their families.
Valid driver's license.
Additional Considerations
Experience in discharge planning and community coordination.
Experience with trauma-informed and recovery-oriented care.
