Job Duties



The ideal candidate will have experience in Direct Patient Care within the limits of Nurse Practitioner’s license: child, adolescent and adult health; family planning; immunizations; travel clinic services; sexually transmitted disease. Thís position will support preadmission screenings to assess patient eligibility and readiness. Assist with Occupational Health activities, including employee health assessments and safety programs. Assist with community-based activities, including vaccine clinics, health screenings, and other outreach initiatives. Support preceptorships by teaching Nurse Practitioner students during their rotations.

The focus and typical core duties are:
Direct Patient Care

  • Provides comprehensive care to patients as assigned per licensed scope of practice and according to local and state policies and procedures; may provide services to the following programs, within the limits of the Nurse Practitioner’s license: child, adolescent and adult health; family planning; immunizations; travel clinic services; sexually transmitted diseases
  • Obtains a complete medical history and records findings in a systematic, accurate and succinct form; reviews patient history and evaluates patient status on current visit; interviews patient for verification of medical history or for full account of present complaints, treatment history, systems review and family/social history
  • Performs accurate physical examinations
  •  Orders, performs, and interprets routine laboratory tests; reviews lab/test results and makes appropriate plan of care changes
  • Administers and prescribes appropriate medications within the guidelines of the practice agreement
  • Develops and implements plans for health promotion and health maintenance, including plans for disease prevention and provision of health education and counseling
  • Provides periodic health assessments of patients
  •  Provides clinical management of patients, co-manages care for patients with selected health complications in collaboration with supervising physician
  • Arranges referrals to other members of the health-care team as needed and/or to appropriate community providers and other community resources

Documents

  • Documents all pertinent findings for clinical exam/labs/diagnostic tests legibly and in a timely manner in standard format per nursing
    protocol and legal requirements; such documentation adequately supports prescribed diagnostic and therapeutic interventions
  •  Uses appropriate VDH forms specific to clinic programs
  •  Develops and records differential diagnosis, problem list and plan of care for patient according to evaluation of all information from history, physical examination and diagnostic tests, and other available information
  • Updates or adjusts care plan subsequently with receipt of additional information
  •  Explains plan of care/findings to patient, documents patient education
  • Refers patients to other providers or agencies according to need and protocol and/or recommendations


Program Evaluation and Planning

  • Assists leadership in program planning and evaluation; contributes to the development and changes in nurse practitioner protocols
  • Assists with evaluation of clinic services and activities; sets and meets goals for client/provider ratio and clinic flow
  • Provides leadership to staff participating in clinic; sets professional example and promotes participation, learning and development of clinical nursing skills: provides training and assists with continuing education for other nursing staff as needed
  •  Suggests changes and additions for services provided within scope of clinic; sets standards and priorities in the clinical setting and assures all applicable local, state and federal protocols are followed
  • Assists in reviewing current health education materials and suggests or makes changes or additions to promote reduction in high risk behavior and positive chronic disease health intervention methodologies


Community Engagement and Other Duties

  • Promotes improved health in the community; assists with health fairs and screening programs as needed
  • May be a resource to community groups or agencies in matters regarding public health; may serve on committees related to community health
  •  May perform other duties as assigned; may be required to assist in the event of an emergency declaration

Minimum Qualifications

  • Demonstrated knowledge of nursing practice and health principles, physical examination techniques and findings both normal and
    pathological, disease states, diagnostic testing and limits of testing, and pharmacology.
  • Ability to perform and interpret physical assessments, obtain medical history, perform and/or interpret diagnostic testing, formulate
    differential diagnoses and plan of care and provide health education to clients.
  • Ability to develop patient management plans; excellent clinical judgement with ability to know the limits of practice and when to seek
    consultation.
  •  Demonstrated ability to work without direct supervision; strong communication and interpersonal skills; ability to work effectively
    with staff, patients and the public; ability to participate in educational and professional programs.

Additional Considerations

  • Prior public health or community health with significant clinical experience in prenatal, STI, family planning, pediatrics and family practice preferred.
