Client Advisor (Case Manager / Care Advisor)

We’re looking for a Client Advisor to manage Home Care Package recipients and other Benetas clients in the Metro East region, working as part of a team to ensure clients have programs / packages based on assessed need and personal goals, and one that promotes their wellness and independence. Some of your general duties will include:

  • Provide packaged and private clients with appropriate information relating to their needs
  • Undertake considered and respectful client intake and assessments, including planning, monitoring and regular reviews
  • Assist clients in understanding of their package spend and the choices available to them
  • Practice within applicable legislation while recognising and respecting the client’s right
  • Actively promote and refer clients to Benetas services
  • Implement best practice quality models of assessment, care plan development, implementation and evaluation

To best support our clients, we’re looking for people with:

  • Tertiary qualifications in allied health or social services-related discipline (essential)
  • Experience in case management of government-funded packages, community resources, and relevant legislation in the provision of aged care services
  • A proven ability to take a holistic approach to client needs and service delivery 
  • Exceptional interpersonal and communication skills to demonstrate empathy and understanding to older clients and their families
  • A positive and proactive approach to working collaboratively within a team and the broader community
  • An understanding of the issues experienced by aged care clients and their carers in the home and the differing social, spiritual and cultural needs of clients
  • Competency in Microsoft Word and Excel programs
  • Ability to provide or obtain a suitability police check outcome, and produce evidence of the 2024 flu immunisation
