The Role:

Benetas Community Health and Care (CHC) provides diverse services to older clients within their homes across Victoria. These services include individual in-home support through Commonwealth funding (CHSP), Support at Home, Veterans Home Care, respite services and private in-home care services. 

Through exceptional customer service and thoughtful advice, our Care Coordinators help our clients to make the best use of their services, enabling them to live at home longer. 

  • Coordinate services in partnership with clients, families, and the Benetas team
  • Support clients with their understanding of budgets and maximise use of available resources
  • Promote independence and informed decision-making by providing clear and relevant information
  • Interpret and apply policies, procedures and legislation to assist with the development of holistic plans
  • Advocate for clients and liaise with external service providers as needed
  • Manage high volume of calls whilst maintaining accurate client records and documentation

About You:

  • A genuine passion for supporting older people to live their best lives
  • Strong communication and problem-solving skills
  • Demonstrated administration skills and methods for staying organised
  • The ability to work both independently and collaboratively in a team
  • Intermediate computer skills and ability to navigate systems
  • Respect for diversity and the ability to build trust with clients and their families
