We’re looking for a Care Partner to join our team and support clients across South Gippsland. This is a rewarding opportunity for someone who enjoys building genuine relationships, coordinating services and making a meaningful difference to the lives of clients and their families.

As a Care Partner, you’ll be a key point of contact for a caseload of clients, helping them navigate their care journey and ensuring their support remains personalised, responsive, and aligned with their goals.

What you’ll be doing

  • Build trusted relationships with clients, families, carers, and representatives.
  • Develop personalised care plans for Support at Home clients and complete holistic assessments.
  • Coordinate a range of services to provide a seamless client experience.
  • Monitor changing client needs, identify risks, and take proactive action.
  • Support clients to understand their care options, budgets and make informed decisions.
  • Manage individual client budgets and ensure services provide value and positive outcomes.
  • Work closely with internal teams, health professionals, service providers, and community partners.
  • Support smooth transitions across care pathways, including restorative care, assistive technology, home modifications, and end-of-life care.
  • Help resolve concerns and ensure safe, high-quality, and compliant care.

What you’ll bring

We’re looking for someone who is person-centred, organised, collaborative and confident working independently.

You’ll ideally have:

  • A Certificate III, IV or higher qualification in aged care, community services, nursing, social work, allied health or a related discipline, or equivalent care management experience.
  • Experience in client assessment, care planning and service coordination within home, community or aged care services.
  • The ability to work collaboratively within a multidisciplinary team and manage competing priorities.
  • Confidence learning and using new systems and technology.
  • Strong communication and relationship-building skills.
  • The ability to make sound decisions, manage risks and adapt to changing client needs.
  • A genuine commitment to supporting older Australians to achieve their goals, independence and quality of life.
  • You’ll also need a current Australian Driver’s Licence, reliable vehicle, Aged Care Worker Screening Check (including satisfactory Police Check) and Working With Children Check.
