The Behavioral Health Care Manager plays a vital role in supporting and coordinating the mental and physical health care of patients enrolled in the OSU-CHS Collaborative Care Program. This position works closely with primary care providers, mental health professionals, and a multidisciplinary team to manage an assigned patient caseload. Responsibilities include conducting patient intakes, delivering brief therapeutic interventions, promoting health literacy, and connecting patients with community resources. |
