Position Introduction:

As a Lead Case Manager, you'll serve as a trusted resource for both consumers and fellow Case Managers, helping ensure exceptional care coordination, operational support, program productivity, and regulatory compliance. Your leadership will help foster a culture of collaboration, accountability, and continuous improvement while supporting the delivery of person-centered services that improve health outcomes and quality of life. Working alongside an interdisciplinary team, you'll help guide complex case management services throughout the continuum of care while mentoring and supporting fellow Case Managers in their professional development. Your expertise will help strengthen clinical decision-making, promote best practices, and ensure consistency in service delivery. Whether providing guidance on challenging cases, assisting with program initiatives, or serving as a liaison with community partners, you'll play an integral role in maintaining a high-performing and consumer-focused program.

This role extends beyond direct service delivery by providing meaningful opportunities to influence program operations and quality. You'll contribute to continuous quality improvement efforts, help ensure compliance with state and federal regulations, and support initiatives that strengthen workflows, documentation standards, and service delivery processes. Your ability to balance clinical excellence with operational oversight will help maintain program effectiveness while ensuring services consistently meet contractual, regulatory, and organizational expectations. A master's degree in Health, Human Services, Social Work, Nursing, Public Health, Counseling, or a related field is strongly preferred, along with additional training or coursework in case management, quality improvement, regulatory compliance, or leadership. Candidates who bring experience in quality assurance, compliance monitoring, service plan review, staff mentoring, or lead-level case management responsibilities will be well positioned for success in this role. Experience working within 1915(c) Medicaid Waiver Home and Community-Based Services (HCBS) programs is highly valued, as is a strong understanding of person-centered planning, regulatory standards, and coordinated service delivery. Required Education and Experience:

  • Bachelor's degree in Social Work, Psychology, Sociology, or other Health or Human Services related field.
  • 24 months experience in case management.

Equivalency/Substitution: Will accept an Associate's degree in Nursing in lieu of the Bachelor's degree.

Required Certifications and Licenses:

  • Social Work (LCSW), Master's Prepared OR Registered Nurse (RN)
  • Certified Case Manager (CCM) (allowed 1 year from hire date to obtain the CCM certification)

Department Preferences:

  • One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population.
  • Certified in Case Management Master's degree in a Health, Human Services, Social Work, Nursing, Public Health, Counseling, or related field.
  • Additional training or coursework in case management, quality improvement, regulatory compliance, or leadership preferred.
  • Experience in quality assurance, compliance monitoring, service plan review, staff mentoring, or lead-level case management responsibilities.
  • Experience working within 1915C Medicaid Waiver home and community-based services programs.
