The Role
- The Allied Health Complex Care Coordination (CCC) team is an integral, highly valued team of multi-disciplinary allied health professionals, applying a collaborative transdisciplinary approach to achieve innovative solutions to complex issues surrounding patients.
- We are seeking an experienced allied health professional to join our team based at Launceston General Hospital, to work alongside nursing, medical and allied health teams, and in close collaboration with external service providers, to support the right place right care patient journey and quality outcomes for our patients.
- The successful applicant will provide expert clinical advice on care management pathways, systems navigation, discharge planning and hospital avoidance for patients with exceptional needs.

Details of Appointment
Fixed term, full time, day worker position, working 76 hours per fortnight, commencing 28th July 2025 until 24th July 2026.
Please note that hours may be negotiated with the successful applicant.
Classification:
Salary $123,717 - $134,336 per annum (pro rata) + 11.5% superannuation + access to salary packaging
- Please note that access to salaries beyond $130,900 is subject to qualifications
- Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement 2022

Eligibility
Successful applicants will be required to meet the essential criteria:
- Satisfactory completion of an approved allied health professional tertiary
qualification/program of study and registered with the relevant National Board
or, in the case of self-regulated allied health professions, full membership/eligible
for membership with the relevant professional association
Applicants should note the following criteria are desirable:
- Holds, or is working towards, post graduate qualifications relevant to the position
