The Team:

The Hospital@home South service plays a vital role in the communities of Southern Tasmania. It offers hospital-avoidance interventions and both acute and subacute care for adults, ensuring safe, effective, multidisciplinary, hospital-level care within the community.

The service operates across multiple sites throughout Southern Tasmania. Hospital@home: South satellite sites are currently located in Cambridge, Glenorchy, and Sorell.

The Role:

The Hospital@home Clinical Nurse Specialist (CNS) is an experienced nurse who provides:

  • Complex care coordination, prioritisation, assessment, and delivery of advanced clinical nursing care to Hospital@home patients within the community setting. 
  • Clinical leadership, expertise and education within the Hospital@home team.

Details of Appointment:

Fixed term part time, shift worker position, working up to 48 hours per fortnight, commencing from 1 June 2026 for approximately five months. 

Please note that hours may be negotiated with the successful applicant

Classification: Registered Nurse Grade 5

Salary: $109,767 - $115,813 per annum (pro rata) + 12% superannuation + access to salary packaging

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023

Eligibility:

Successful applicants will be required to meet the essential criteria:

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

  • Current Driver’s Licence

Applicants should note the following criteria are desirable:

  • Postgraduate qualification in Community Nursing or working towards a relevant postgraduate qualification

Under particular circumstances employees may be required to undertake a new conviction check.
