The Role

The Rural and St Johns Rehabilitation Liaison CNC service operates from the Royal Hobart Hospital (RHH) Integrated Operations Centre, supporting acute and subacute services across the Tasmanian Health Service (THS) Southern region. The team provides a high‑level consultancy service to identify patients suitable for rural hospital admission or rehabilitation beds and to ensure care occurs in the most appropriate setting.

Working closely with inpatient units, rural facilities, community services, and rehabilitation providers, the team coordinates complex discharges and facilitates safe, timely transfers to sites such as NNDH, Oatlands, Esperance, Tasman, May Shaw, and St Johns Rehabilitation.

The team strengthens continuity of care across the acute–subacute–community continuum through clinical assessment, advice, and strong communication networks. They also contribute to quality, safety, and system‑wide improvement by supporting effective patient flow and enhancing coordinated transitions for patients with complex needs.

Details of Appointment

Fixed term, part time, shift worker position, working 64 hours per fortnight, commencing from 24 August 2026 until 18 May 2027.

Classification:

Registered Nurse, Grade 6

Salary:

$118,297 - $125,543 pro rata per annum + 12% superannuation + access to salary packaging

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023

Eligibility

Successful applicants will be required to meet the essential criteria:

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

Applicants should note the following criteria are desirable:

  • Holds, or is working towards, a relevant tertiary qualification and/or recent extensive nursing experience relevant to the role

Under particular circumstances employees may be required to undertake a new conviction check.
