The Role

• The Clinical Nurse Consultant - Care@home will:

  • Identify hospital patients ready for discharge and coordinate their timely transfer into the Care@home program.
  • Provide clinical leadership and expert advice using current evidence-based healthcare practices.
  • Support the Nurse Manager with daily operations to meet the program's strategic goals.
  • Act as the lead clinical resource and mentor to guide hospital staff through virtual care referral pathway.

Details of Appointment

Multiple fixed term full time, day work, working up to 76 hours per fortnight, commencing as soon as possible for approximately 12 months.

Positions open in the following Regions:

Launceston: Launceston General Hospital (LGH).

North West: North West Regional Hospital (NWRH) and Mersey Community Hospital (MCH).

Classification Registered Nurse Grade 6

Salary

$118,297 - $125,543 per annum (pro rata) + 12% superannuation + access to salary packaging

• Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023

Eligibility

Successful applicants will be required to meet the essential criteria:

• Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

Applicants should note the following criteria are desirable:

• Holds, or is working towards a relevant postgraduate qualification

Under particular circumstances employees may be required to undertake a new conviction check.
