Overview

The Clinical Nurse (Respiratory Inpatients & Home Oxygen) provides nursing services as part of the Adelaide Hills Community Nursing Directorate.

Please note: working 3.8 hours per week as a Clinical Nurse for Respiratory Inpatients and working 15.2 hours per week in the Home Oxygen program.

We are looking for a proficient and responsible Clinical Nurse to join our welcoming team in Country Health Connect. The role provides the delivery of the SA Home Oxygen program including Assessment, care planning, evaluation and review, to clients living in the Adelaide Hills. Additionally the role supports the education, self-management and rehabilitation of patients with respiratory illness to best manage their health condition. It will contribute to reducing emergency department admissions, and length of hospital stay for patients admitted with respiratory illness to Mt Barker Hospital in addition to supporting timely transfer of respiratory patients from Metropolitan Health services to their community or residence.

The Clinical Nurse is actively involved in the planning, facilitation and evaluation of service specific Health Promotion activities for these clients and affiliated community groups.
