We are seeking a dedicated and skilled Registered Nurse to join our Hospital @ Home Team.

Hospital @ Home is a dedicated nursing service based at Flinders Medical Centre (FMC) which provides acute nursing care and support to patients in their own home, including on their return home from hospital after an illness. This care is a substitute for treatment at the hospital, meaning people can return home sooner or avoid hospitalisation altogether.

About the Role:

As the Registered Nurse, you duties may include: 

  • Providing direct person-centred nursing/midwifery care and/or individual case management to patients/clients on a shift by shift basis
  • Assessing individual patient needs, planning, implementing and coordinating appropriate service delivery from a range of accepted options including other disciplines or agencies.
  • Providing treatment that would otherwise be provided in hospital.
  • Recognising and responding to clinical deterioration or other incidents and escalating appropriately

To be successful in this role, you will need: 

  • Advanced assessment skills, advanced wound management experience
  • Knowledge and experience with PICC and ports, venepuncture, IV cannulation
  • Effective communication skills including, problem solving, conflict resolution and negotiation skills including the confidence to work autonomously in the community.
  • A current Australian drivers licence
