Overview

The NDIS Package Coordinator is responsible to the Program Manager, NDIS and Community Connections for the coordination and provision of NDIS service agreements within the Eyre & Far North Local Health Network

Key responsibilities:

  • Ensuring that NDIS targets are met and that eligible clients receive appropriate services to assist with reaching their identified plan goals and to ensure that services offered are culturally appropriate and inclusive
  • Monitor, promote and foster a culture centred on customer service. You must provide a high level of empathy and support to clients and their families to ensure that service provision is efficient and effective within the NDIS Space.
  • Work closely with Allied Health clinicians, Allied Health Assistants, specialist Nursing services and administration within Country Health Connect as well as other external service providers as per the client’s service plan
  • Possess the ability to work independently, maintain program information and be able to monitor budgets, produce reports and actively promote Country Health Connect as a provider of choice within the NDIS services.

Location is negotiable working across our sites in Cleve, Cowell, Kimba or Wudinna

This is a part time vacancy, working 37.5 hours per fortnight and salary will be adjusted according to actual hours worked.
