About the Role

Are you a passionate and experienced allied health clinician ready to take the next step in your career? Join our dynamic Central Adelaide Local Health Network (CALHN) team as a Senior Discharge Coordinator, where you’ll work closely with a dedicated multi-disciplinary team to support the care progression and discharge planning of patients with complex needs. In this pivotal role, you’ll lead the coordination of discharge for patients on the National Disability Insurance Scheme (NDIS) pathway and those facing significant barriers to discharge—ensuring timely, safe and person-centred transitions from hospital to home or community services.

As a senior clinical leader, you’ll provide expert consultation, comprehensive assessments and strategic interventions to drive best-practice discharge planning. You’ll also play a key role in shaping and delivering education and training across CALHN, building staff capability and maintaining a consistent, centralised approach to NDIS-related care. Your knowledge and leadership will influence quality improvement activities across the NSR program, making a real impact in the lives of patients and their families while contributing to an innovative and collaborative team environment.

About You

You are a passionate and experienced allied health professional who thrives in a leadership role, with a strong commitment to delivering high-quality, person-centred care. You’re driven by collaboration, innovation, and achieving the best possible outcomes for patients with complex needs. With your expertise in discharge planning and in-depth knowledge of the NDIS, you are ready to take on a role that blends clinical excellence, strategic planning, and service improvement.

To be successful in this role, you will have:

  • A relevant degree and current registration or eligibility for membership with one of the following: Physiotherapy, Occupational Therapy, Psychology, Social Work, or Speech Pathology Australia.
  • Extensive experience in discharge planning and care coordination for patients with complex health conditions, neurological and orthopaedic presentations, and disability support needs.
  • Strong leadership and clinical consultancy skills, with the ability to mentor others, drive quality improvement initiatives and contribute to policy and service development.
  • Highly developed communication, organisational and problem-solving abilities, with the confidence to advocate, lead multidisciplinary collaboration and deliver effective education and training.
