The Senior Occupational Therapist participates as a member of the Residential Aged Care Facility (RACF) In-Reach Service, working closely with Geriatricians and the Nursing team to deliver care to clients within residential aged care facilities and prevent unnecessary hospital transfers. The role supports clients with memory loss and Behavioural and Psychological Symptoms of Dementia in their residential care environment to develop and action patient centred management plans to support patient's well-being, and support integration of care between SALHN and the facility.

About the Role:

Reporting to the Advanced Nurse Unit Manager, CARE Service and Director Occupational Therapy (DivRAP), duties for the role may include:

  • Assessing clients’ occupational abilities, problems and needs using a variety of formal and informal assessments
  • Devising patient programs and behavioural support plans and supervising residential care staff in their implementation
  • Communicating with clients family, carers and other service providers and providing education regarding ongoing management
  • Documenting assessment findings and interventions in accordance with service standards

About you:

  • High levels of responsibility and organisational skills including the ability to prioritise work and meet deadlines
  • A degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia
  • Experience as a clinician in the assessment and provision of high quality Occupational Therapy services to clients with a variety of conditions requiring management of issues associated with cognitive impairment and/or changed behaviours
  • Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
