About the Role 

Join our dynamic team as the Community Clinical Rehabilitation Coordinator (CCRC) and play a pivotal role in transforming the lives of individuals within the CALHN Community Mental Health Clinical Program (MHCP). As the CCRC, you'll lead the day-to-day coordination of clinical rehabilitation services, ensuring that every consumer receives high-quality, evidence-based care focused on recovery. Your collaborative spirit will shine as you work closely with hospital and bed-based services, facilitating timely access to rehabilitation and crafting effective pathways for care. You’ll be at the heart of a vibrant team culture, promoting teamwork and consumer-centred approaches that empower our clients and embrace the invaluable contributions of the Lived Experience Workforce.

In this leadership position, you'll be responsible for triaging referrals, monitoring staff workloads, and guiding the development of clinical rehabilitation group programs that inspire and engage consumers. Your expertise will help identify rehabilitation needs early on, ensuring that our services are tailored to each individual's journey. By fostering partnerships with various agencies, you'll support consumers in building the skills they need to lead fulfilling lives. About You 

We’re looking for a passionate professional with a strong background in mental health to join our team. You’ll bring a unique blend of qualifications and experience that will drive our clinical rehabilitation services forward.

  • Qualifications: Hold a relevant degree for registration as an Occupational Therapist, Social Worker, or a Master’s in Clinical Psychology.
  • Leadership Skills: Proven ability to lead multidisciplinary teams and manage competing priorities effectively.
  • Experience: Significant experience in mental health settings, particularly with complex cases and rehabilitation services.
  • Communication: High-level interpersonal skills to build strong partnerships with consumers, families, and stakeholders.
