Role Overview:

As the Community Services Coordinator, you will:

  • Report to the Regional Community Health Team Leader and coordinate Home Support programs within Coober Pedy.
  • Ensure compliance with Home and Community Care Under 65yrs, Commonwealth Home Support, Commonwealth Home Care, and Disability SA/NDIA targets.
  • Provide appropriate services to eligible clients, assisting them in maintaining independence in their homes while ensuring cultural appropriateness and inclusivity.
  • Utilise a primary health care approach to implement health promotion strategies, including illness prevention, health education, community development, advocacy, and social action.
  • Foster a customer-centered culture, promoting teamwork and ensuring services address the needs of minority and disadvantaged individuals.

Key Responsibilities:

  • Coordinate and oversee Home Support programs to meet all relevant targets and service standards.
  • Implement health promotion strategies and community development initiatives.
  • Ensure services are delivered in a culturally sensitive and inclusive manner.
  • Advise medical, nursing, and allied health personnel in the Coober Pedy area on home and community care.
  • Monitor and promote a culture of excellent customer service and effective teamwork.
  • Address the needs of minority and disadvantaged groups through service provision and employment practices.
