About the Role:
As a Nurse Practitioner or Nurse Practitioner Candidate, you will play a key role in delivering advanced clinical care. This includes assessing, diagnosing, treating, and managing acute and chronic conditions, prescribing medications, ordering diagnostic tests, and referring patients to specialists. You will be instrumental in improving health outcomes through a strategic, patient-centred approach, with a focus on addressing complex care needs and reducing hospital admissions.

This is an exciting opportunity to work in primary care settings, particularly targeting populations such as those with chronic conditions, frail elderly, individuals with mental health challenges, and others with complex or underserved needs. The role also includes contributing to quality improvement, engaging in collaborative learning, and using digital health tools to enhance patient care. Work hours will be Monday to Friday and we offer negotiable flexible work arrangements. 

Key Responsibilities:

  • Conduct advanced health assessments and make informed diagnoses
  • Prescribe and manage pharmacological treatments
  • Order and interpret diagnostic investigations
  • Coordinate patient care, referrals, and community-based services
  • Support the development of innovative care models within the practice
  • Scope of practice to be negotiated

Please note this role is multi classed as RN/RM3 or RN/RM4, and classification will be determined by qualifications and experience
