We’re looking for a dedicated Aboriginal Case Manager for our Home Care Package (HCP) Program. About the Role:

The Aboriginal Enrolled Nurse Case Manager (ENC-END) – Home Care Package Program provides high-quality, home-based care to consumers with complex needs. The role is focused on supporting individuals to live independently in their chosen environment, offering flexibility and choice in how care and support are delivered. Working collaboratively with consumers, families, and carers, you will develop and implement personalized care plans, using your professional judgment to tailor service delivery within budget and risk parameters. You will also support the Registered Nurse/Midwife in providing person-centred care while adhering to regulatory standards.

  • Deliver high-quality home-based care to complex HCP consumers.
  • Work with consumers, families, and carers to create personalized care plans.
  • Utilize professional judgment to design tailored solutions within budget and risk parameters.
  • Support the Registered Nurse/Midwife in providing person-centred care.
  • Practice at novice to proficient Enrolled Nurse levels with responsibility for actions.
  • Contribute to improving Aboriginal health outcomes in line with Closing the Gap initiatives.

The Aboriginal Case Manager (OPS2) – Home Care Package Program is responsible for providing high-quality home-based care that supports individuals to live independently in their own homes. The role focuses on offering choice and flexibility in the delivery of care, using a strengths-based, goal-directed approach to ensure each consumer’s needs are met. You will collaborate with consumers, their families, and carers to develop care plans, tailoring services to meet individual requirements while working within budget and risk constraints.

  • Provide home-based care to HCP consumers, supporting independence and flexibility in care.
  • Take a strengths-based, goal-directed approach to care planning and service delivery.
  • Work with consumers, families, and carers to identify goals and develop care plans.
  • Tailor service delivery based on consumer’s needs, within budget and risk parameters.
  • Design flexible care solutions using professional expertise and judgment.

What We’re Looking For:

ENC/END

  • Enrolled or eligible for enrolment as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
  • Experience in the provision of nursing care in the healthcare setting in accordance with the appropriate standards of practice.

OPS2

  • Experience in assisting with and promoting social inclusion.
  • Experience in consumer directed care.
  • Aged care experience.
  • Experience relevant to working in a community setting.
  • Experience in the use of personal computers and computerised client information systems

Applicant Information

This is a part time vacancy, working 46 hours per fortnight and salary will be adjusted according to actual hours worked.

Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements:

  • DHS National Disability Insurance Scheme (NDIS) Worker Check
  • National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
  • Immunisation Risk for this position is - Category A
