Overview

Are you a passionate and experienced nurse looking to make a meaningful impact in chronic condition management? Join our team as a Chronic Condition Nurse (ANUM) and play a pivotal role in enhancing health services across regional South Australia.

The Chronic Condition Nurse (ANUM) will be required to support the implementation of effective and efficient chronic condition services across regional South Australia to:-

  • Reduce emergency department presentations and hospital admissions for clients with chronic and/or complex conditions. 
  • Reduce the length of stay for patients with chronic conditions admitted to regional public hospitals. 
  • Reduce patient journeys to access health services.
  • Timely transfer of patients with chronic conditions from metropolitan health services to their community of residence. 
  • Increased effective use of health services by vulnerable groups.

The Chronic Condition Nurse (ANUM) is accountable for the delivery of high-quality nursing care which is consistent with the Department of Health quality framework. Employees in this role accept accountability for the outcomes of nursing and or midwifery practices in the specific practice setting, for addressing inconsistencies between practice and policy; and for developing team performance and a positive work culture in the interest of patient/client outcomes.

  • Conducting daily monitoring of clients supported through the VCC Service. 
  • Providing expert chronic condition advice and support to consumers utilising VCC.
  • Harness digital technologies for client service delivery.
  • Precisely manage database systems for recording client information and reporting of data. 
  • Assist in the implementation of new systems to support the effective utilisation of VCC services and other eHealth services to support clients with chronic conditions.
  • Provide clinical and program support to other regional chronic condition services as required.

Qualifications and Experience:

  • Registration or eligibility for registration as a Nurse with the Nursing and Midwifery Board of Australia.
  • Current practicing certificate or eligibility to obtain one.
  • Demonstrated expertise in chronic condition management and program implementation.
  • Strong communication and leadership skills with a commitment to quality improvement.
