As a Community Support Worker in our innovative Consumer Directed Care model, you will provide personalised care to people who are aged, frail, or living with a disability, helping them maintain independence and stay in their homes or Residential Aged Care facilities.

In this role, you will deliver vital services like personal care, meal preparation, cleaning, transport, and social support. You'll work closely with consumers, their families, and health professionals, helping them make decisions about their care while ensuring quality and compassionate outcomes. Flexibility, empathy, and the ability to work autonomously are key to success in this role.

Key Criteria:

  • Experience in providing direct care services in a health-related field.
  • Experience working with aged, frail, or disabled individuals.
  • Ability to implement detailed care plans and follow instructions.

Tasks may include:

  • Personal care (showering, toileting, grooming, continence management).
  • Household tasks (cleaning, meal preparation, laundry, minor gardening).
  • Social support (transport, shopping assistance, mobility support).
  • Assisting with self-medication under clinical supervision.
  • Ability to work autonomously while collaborating with consumers, families, and health professionals.
