About the Role

Are you passionate about providing high-quality care to vulnerable populations? As an Enrolled Nurse within the CALHN Integrated Care Service, based at the Refugee Health Service, you will play a key role in delivering intervention, assessment, care planning, and education within your scope of practice. Working collaboratively in a multidisciplinary team, you will ensure patients’ post-discharge needs are met by coordinating care plans, arranging follow-up appointments with general practitioners (GPs), and educating patients about medications, self-care, and follow-up care.

This role involves supporting hospital avoidance initiatives and addressing barriers to healthcare access for refugees. You will work closely with GPs, nurses, allied health professionals, and external organisations to develop personalised discharge plans, coordinate referrals, and strategise ways to ensure successful engagement with primary care services. About You

We are looking for a dedicated and adaptable nursing professional with strong communication skills and a passion for providing exceptional care. To excel in this role, you will bring:

  • Strong Communication and Problem-Solving Skills: Effective verbal and written communication, IT literacy, and the ability to resolve conflicts, solve problems, and negotiate effectively.
  • Clinical Expertise and Multidisciplinary Teamwork: Demonstrated competence in nursing practice, accredited in venepuncture, and proven ability to work collaboratively within a multidisciplinary team.
  • Organisation and Adaptability: Self-directed with the ability to manage time effectively, prioritise workloads, maintain accurate records, and adapt creatively to meet the changing needs of clients in a clinical or community setting.
  • Knowledge of Diverse Healthcare Needs: Understanding of complex health issues, healthcare standards, and considerations for vulnerable populations, including migrants, Aboriginal and Torres Strait Islander clients, and those experiencing homelessness or disabilities.
