About the Opportunity

We currently have an exciting opportunity to join the Metropolitan Youth Health (MYH) team as a Youth Health Worker, where you will be responsible for:

• Engagement of young people who are disadvantaged and/or vulnerable to access and navigate through specialised primary health care services
• Development and delivery of quality outcomes for clients through the provision of youth health care services within a case management framework, aligned to the Metropolitan Youth Health Model of Care

About You

To be successful in this role, you will have the ability to:

• critically analyse and respond to the changing needs of the clients and the organisation.
• lead collaborative work activities, prioritise workloads and meet set timelines, whilst working under minimal supervision.
• develop and deliver training and development programs
• provision of a high standard of assessment, case planning and health care services within a case management approach to young people with complex needs
