About the Role:

Are you a passionate and dedicated healthcare professional seeking a rewarding career opportunity? The Northern Adelaide Local Health Network is currently seeking a Clinical Care Coordinator to join our Northern Community Mental Health team.

The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.

In this vital role, you will play a crucial part in ensuring the delivery of high-quality, person-centred care to individuals experiencing mental health challenges. As a Clinical Care Coordinator, you will work collaboratively with a multidisciplinary team to coordinate and monitor the care and recovery journey of consumers, fostering a supportive and empowering environment.

Key responsibilities will include:

  • conducting comprehensive mental health assessments and developing individualized care plans
  • facilitating seamless transitions between various care settings, ensuring continuity of care
  • liaising with consumers, families, carers and external service providers to promote integrated service delivery
  • providing psychosocial education and support to consumers and their support networks
  • advocating for consumer rights and promoting recovery-oriented practices

To be eligible for this position as an Allied Health Professional, you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically:

  • Appropriate degree (Master of Clinical Psychology) or equivalent qualification recognised by the PsyBA. Must hold general registration with the PsyBA and be endorsed in Clinical area of practice. Must hold approved supervisor status with the PsyBA at all levels of supervision (primary and secondary) OR
  • Appropriate degree or equivalent qualification which gives eligibility for full membership with the Australian Association of Social Workers OR
  • Appropriate degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Australia.

To be eligible for this position as a Registered Nurse, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate. Must be enrolled in an approved Mental Health course or holds a qualification in mental health practice.
