Lead with Purpose as a Team Leader – Optimal Ageing: Shape Innovative Community Health Services

Are you a seasoned healthcare professional ready to lead, inspire, and make a real difference in the lives of older community members? We’re seeking a dynamic and experienced Team Leader – Optimal Ageing to coordinate and drive high-quality, client-centered care across a multidisciplinary team of dedicated health professionals.

In this vital leadership role, you’ll oversee the day-to-day delivery of community-based services, while also playing a key role in operational planning, service evaluation, and continuous quality improvement. Your clinical expertise, combined with your leadership skills, will support a collaborative and responsive approach to ageing well in the community.

As a Team Leader – Optimal Ageing, you will:

  • Provide strong operational leadership and coordinate a team of health professionals across diverse service models.
  • Monitor service delivery, lead data collection and evaluation, and ensure services align with community needs.
  • Contribute to service planning and integration across programs within the Community & Allied Health Directorate.
  • Foster workforce development, positive team culture, and high standards of care through coaching and collaboration.
  • Be accountable for quality outcomes, service innovation, and aligning clinical practice with strategic goals.
