We are seeking a dedicated and skilled Social Worker to join our Transition Services Team. 

About the role: 

As the Social Worker you will be working within the Care Awaiting Placement South (CAPS) team where you will assist the implementation and management of this service delivery model. Duties for the role will include:

  • Developing, implementing and evaluating innovative responses and flexible service options to meet identified needs to support the CAPS program
  • Receiving and triaging referrals from the acute and sub-acute sectors
  • Providing support for both the acute and community service providers in operational aspects of the CAPS Program
  • Being a resource for patients, staff, families, service providers and other stakeholders regarding the CAPS Program

About you:

  • Effective liaison, goal setting and negotiation skills and ability to work in a flexible manner
  • Demonstrated skills in case management/care coordination and the ability to make complex clinical decisions including sourcing and coordinating services to meet clinical and community care needs
  • Demonstrated experience working within a multi-disciplinary team with contribution to the development and attainment of team goals
  • A degree in Social Work (eligibility for membership of the AASW)
  • Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
