About the Role

 Are you passionate about supporting older people to live independently and thrive in the community after hospital discharge? The Transition Care Program (TCP) is looking for a dynamic and compassionate AHP2 Coordinator to join our dedicated team. In this rewarding role, you will be at the forefront of helping older individuals make a safe and supported transition from hospital to the next stage of their care. You’ll work closely with multidisciplinary teams to identify suitable clients within CALHN inpatient services, coordinate their discharge to appropriate rehabilitation or community supports, and develop short-term care plans that promote recovery and independence.

As the TCP Coordinator, you will play a vital role in shaping a seamless journey for each client. You'll liaise with hospitals and community services, establish strong service linkages across aged care and primary health networks, and ensure ongoing case management is in place to support each individual's unique needs. With a focus on collaboration, accountability, and continuous improvement, you'll contribute to high-quality outcomes while mentoring junior staff and maintaining professional standards of practice. About You

You are a dedicated and experienced Allied Health professional with a passion for supporting older people to live independently and with dignity. You bring clinical expertise, strong communication skills, and a collaborative approach to delivering high-quality care in a multidisciplinary environment.

  • Hold an appropriate Allied Health degree with relevant registration or eligibility

  • Minimum 2 years post-graduate experience, ideally in aged care or restorative programs

  • Skilled in assessment, care planning, and linking clients with community services

  • Strong interpersonal, time management and problem-solving skills, with a client-centred focus
