Are you a skilled and compassionate Registered Nurse looking to make a difference in the lives of individuals within a community setting? We’re seeking a dedicated Community Nurse to join our team and provide high-quality, person-centred care. This role offers an opportunity to grow as you work alongside a collaborative team in a supportive, dynamic environment.

Key Responsibilities:

  • Assess patient needs and plan, implement, and coordinate care services, working with multidisciplinary teams to ensure comprehensive care.
  • Provide direct nursing care, managing individual cases and coordinating services to meet health needs.
  • Lead shifts and coordinate team activities to ensure safe, effective patient care.
  • Offer health promotion and education to patients, clients, and carers, improving health outcomes within the community.
  • Contribute to quality improvement initiatives, ensuring patient safety and optimal care standards.
  • Support the development of nursing practice by guiding less experienced staff and students.
  • Participate in research activities and apply evidence-based guidelines to enhance patient care.

What We’re Looking For:

  • Registered Nurse with current registration or eligibility for registration with the Nursing and Midwifery Board of Australia.
  • A minimum of 2 years post-graduate experience, with demonstrated competence in nursing practice.
  • Strong communication skills, including problem-solving, conflict resolution, and negotiation.
  • Ability to work effectively within a multidisciplinary team, and adapt flexibly to healthcare challenges.
  • Commitment to consumer and family-centred care, with excellent time management skills.
