The Northern Adelaide Local Health Network is seeking an experienced Social Worker to join our team within the Aged Care, Rehabilitation and Palliative Care division. This position offers the opportunity to make a profound impact on the lives of individuals and communities.

As the Senior Social Worker Lead / Senior Social Worker Clinician, you will provide professional leadership and coordination to support the Aged Care Rehabilitation and Palliative Care Division (ACRPC). In collaboration with the Nurse Unit Manager/Senior Manager, ACRPC Allied Health Lead and Social Work Senior Manager NALHN, the Social Work Lead will ensure appropriate professional governance and compliance across the division.

In this role, you will play a pivotal role in providing comprehensive social work services to patients and their families. Your responsibilities will include conducting assessments, developing and implementing care plans, facilitating discharge planning and collaborating with multidisciplinary teams to ensure holistic and patient-centred care.

In addition to your primary duties, you may be required to perform other related tasks, including participating in quality improvement activities, research projects and contributing to the development and implementation of policies and procedures.

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
