Are you a passionate and highly skilled Nurse Practitioner seeking an opportunity to make a profound impact on the lives of the Kangaroo Island community? SA Health is seeking a dedicated Nurse Practitioner for a Part-time, Temporary Contract position within the Kangaroo Island Outreach Program, located in the BHFLHN Kangaroo Island Community & Allied Health unit, under the Barossa Hills Fleurieu Local Health Network.

As a Nurse Practitioner endorsed by the Nursing and Midwifery Board of Australia, you will be an expert practitioner, practicing beyond the usual extent of a nurse, at an advanced level, with an expanded scope of practice and an advanced degree of autonomy and clinical decision-making. You will work independently and collaboratively within the healthcare system, providing high-level clinical nursing expertise for specified individuals, groups, and populations.

In this role, you will lead the Kangaroo Island Outreach Program, delivering primary healthcare services that respond to the complex health and chronic disease needs of the local community. You will work across various settings and locations, including outreach clinics and home visits, focusing on chronic disease health management, health promotion and wellbeing, care coordination, and disease prevention.

Your responsibilities will encompass providing customer-focused services incorporating advanced clinical nursing care, assessment, triage, coordination, education, consultancy, research, management, and mentoring. You will collaborate with medical and other healthcare professionals, contributing to safe, efficient, and best-practice patient care within your defined scope of practice.

To be considered for this exceptional opportunity, you must possess a Master's Degree in a relevant specialty area (Primary Health Care/Chronic Disease) and be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia. Additionally, you must hold or be eligible to hold a current practicing certificate with the notation of Nurse Practitioner and be endorsed as a Nurse Practitioner with AHPRA.
