About the Opportunity

We currently have an exciting opportunity to join the Metropolitan Youth Health team as a Community Nurse, where you will be responsible for:

  • Deliver advanced, client-centred nursing care to vulnerable young people, including those under Guardianship of the Minister, within a multidisciplinary team and the MYH Model of Care.
  • Lead and coordinate case management, including assessment, screening, referrals, treatment, and psychosocial support services.
  • Provide clinical leadership and mentoring to students, new staff, and less experienced colleagues, fostering a culture of learning and development.
  • Engage young people in health education and self-management projects while advocating for equitable access to primary health care services.
  • Contribute to service evaluation, data analysis, and quality improvement initiatives to enhance nursing practice and care outcomes.
  •  Promote culturally safe, inclusive care and actively support partnerships with clients, families, and external agencies to improve service access and delivery.

 

About You

To be successful in this role, you will have:

  • Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia (NMBA) and who holds, or who is eligible to hold, a current practicing certificate.
  • Well-developed interpersonal and communication skills including the ability to work with individuals and families for positive health outcomes and to foster productive working relationships across disciplines.
  • Proven ability to provide clinical nursing health care services within a clinical health setting.
  • Demonstrated ability to relate to people from a diverse range of cultural and social background.
  • Registered Nurse and Midwife with at least 3 years post registration experience.
  • Demonstrated competence in the relevant area of nursing practice.
  • Experience in problem solving, negotiation and conflict management.
