About the role
We are seeking a motivated and compassionate Physiotherapist to join the Care of the Older Person and Community Transition (CO-ACT) service, based at Hampstead Rehabilitation Centre. In this rewarding role, you will provide physiotherapy assessment and intervention to patients with a wide range of needs, contributing to their recovery and supporting their transition back to the community. Working within a multidisciplinary team, you will play a key role in delivering coordinated, patient-centred care.

You will work with a high level of independence, managing a caseload that includes high-acuity and complex presentations. This position offers an excellent opportunity to further develop your advanced clinical skills in a supportive environment. You may also contribute to quality improvement initiatives and research activities, helping to shape best practice and enhance patient outcomes across the service.

About You

With the ability to work both independently and collaboratively, you bring a flexible and proactive approach to supporting patients in complex and evolving healthcare settings.

You will also bring:

  • Current registration with the Physiotherapy Board of Australia and demonstrated experience in acute hospital or rehabilitation settings.

  • Well-developed clinical reasoning and decision-making skills, with the ability to manage complex caseloads under reduced supervision.

  • Excellent communication and interpersonal skills, with a strong team focus and the ability to work effectively with patients and colleagues from diverse backgrounds.

  • A proactive and adaptable mindset, with a commitment to continuous learning, service improvement, and contributing to seven-day service delivery as required.
