Support Independence and Choice – Community Support Worker Opportunities

We are seeking dedicated Community Support Workers to join our team and help deliver person-centred services across the Eyre and Far North Local Health Network under our Consumer Directed Care model.

In this rewarding role, you will support older people, individuals with disability, and those who are frail to remain in their homes or residential facilities by providing essential services such as personal care, cleaning, meal preparation, transport, and social support. Working closely with each consumer, their families, and health professionals, you will help achieve personalised goals and promote independence.

Under the Consumer Directed Care model, consumers play an active role in deciding the services they receive, who provides them, and when. As a Community Support Worker, you will need to be flexible, empathetic, and confident working autonomously while remaining focused on delivering responsive, high-quality care.

What We're Looking For:

  • Certificate 3 in Individual Support (Ageing, Disability or Home and Community).
  • Current drivers’ licence and willingness to drive.
  • Current police clearances (refer police check clause).
  • Contactable by telephone.
  • Access to a reliable, registered and insured (minimum of third-party insurance) motor vehicle.

Applicant Information

This is a part time vacancy, working 38 hours per fortnight and salary will be adjusted according to actual hours worked.
