Sub-Acute Triage Hub Clinical Nurse - RN2

Join SALHN as a member of the Division of Rehabilitation, Aged Care and Palliative Care Sub-Acute Triage Hub.  A Clinical Nurse role is available and is located in our triage hub at the Flinders Medical Centre site.  The role is responsible for reviewing referrals and assessing suitability of patients for admission to the Rehabilitation and Aged Care services.  The Clinical Nurse role is also involved in contributing to the reporting and quality monitoring mechanisms of the sub-acute triage program. About the role: 

Reporting to the Advanced Nurse Manager, some of your duties for the role will include:

  • Reviewing clinical documentation and assessing patient suitability for the different sub-acute services 
  • Provide education and support to patients and the clinical teams on the sub-acute services available
  • Support the other sub-acute triage hub team members
  • Provide proficient, person centred, clinical nursing care and/or individual case management to patients/clients in a defined clinical area
  • Monitoring patient/client care plans to ensure appropriate care outcomes are achieved 
  • Manage a Safety and Quality portfolio in line with the National Safety and Quality Health Care Standards 

About you:

  • Ability to build rapport and effectively communicate with older adults, their carers and family members
  • Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses
  • Ability to be able to rapidly respond to changing workloads and priorities
  • Ability to be creative, innovative and flexible when approaching issues within the clinical setting

To be considered for the RN2C (Clinical Nurse) level you must have at least three years, full time equivalent, post registration experience with experience in the leadership and direction of student nurses, enrolled nurses, and less experienced registered nurses.
