SALHN is looking for a Nurse Consultant to join their Rehabilitation, Aged Care and Palliative Care Team in an ongoing role to support the patient journey into Sub-Acute services!

As the RAP Triage Nurse Consultant, you will provide clinical nursing expertise for the assessment and triage of new referrals and effective co-ordination of Rehabilitation and Aged Care sub-acute admissions, collaborating with all referring agencies in the promotion and utilisation of Rehabilitation and Aged Care Services in the Southern area of Adelaide.  You will also coordinate and support patient flow processes within the division.

About the role: 

As the RAP Triage Nurse Consultant, duties for the role will include:

  • Contribute expert nursing/midwifery assessment and advice to local clinical teams to achieve integrated nursing/midwifery care within a risk management framework.
  • Be required in a multidisciplinary primary health care setting to apply nursing/midwifery expertise to assess clients, select and implement different therapeutic interventions and/or support programs and evaluate patient/client progress.
  • Identifying hazards, assessing risks and implementing, monitoring and maintaining hazard control measures.
  • Provides leadership and direction, acts a role model, mentor, consultant and resource person
  • Support patients to access the services whilst also managing patient flow within the division of RAP
  • Coordination and leadership of projects and/or programs that contribute clinical expertise to improve patient/client/service outcomes

About you:

  • Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills.
  • Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate
  • Comprehensive patient assessment skills and demonstrated ability to work autonomously
  • Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
  • Demonstrated competence in the relevant area of nursing and or midwifery practice in accordance with the relevant standards. Experience in the supervision of students, enrolled nurses and less experienced registered nurses and or midwives
  • Registered Nurse and or Midwife with at least 3 years post registration experience
