About the Role:

The Northern Adelaide Local Health Network is seeking Social Workers to support patients and families across our Aged Care, Rehabilitation and Palliative Care (ACRPC) Division.

The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.

In this role you'll provide crucial psychosocial support, discharge planning and family counselling. This diverse role offers exposure to complex cases while contributing to improved patient outcomes and quality of life.

Social workers in ACRPC have diverse roles and support client cohorts in speciality areas, respect the diverse nature of the social work role and the specialty areas within ACRPC, including but not limited to:

Palliative Care: providing social work support to palliative care patients, their families and carers across illness and bereavement phases.

Geriatrics: providing social work support to older adults, families and carers, going through life transitions, through an understanding of geriatric conditions and best practice guidelines around management, assessment, intervention, case management and referrals eg. ACAT, MAC, TCP and SACAT.

Rehabilitation: providing social work support to adults participating in rehabilitation. This may involve assessment and intervention for adults going through complex life transitions, grief and loss and needing support for complex psychosocial situations; specialised assessment based on diagnosis eg. stroke and amputee; or referrals to services including NDIS, SACAT, ACAS, MAC and TCP.

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
