Join Our Team as a Community Care Coordinator – Empower People to Live Independently

Are you passionate about helping people stay in their homes and communities for longer? This role offers the opportunity to support ageing and disability clients to live independently while ensuring they have real choice and control over their care.

About the Role:

As a Community Care Coordinator, you will play a key role in the delivery of aged care, disability and home support services that empower consumers to live independently with dignity and choice. You will ensure programs are coordinated and delivered in line with the philosophies, aims and vision of RMCLHN, while managing program budgets and meeting financial and reporting requirements. Your responsibilities include maintaining up-to-date policy knowledge, overseeing program waiting lists, ensuring smooth program operations, and interpreting eligibility criteria to connect consumers with appropriate services. You will support assessments with clients and carers in accordance with wellness and reablement principles, oversee brokerage and contractual arrangements, and liaise with relevant teams to ensure accurate data collection and reporting.

This role also involves representing RMCLHN through engagement with Commonwealth services such as My Aged Care and My Place, as well as attending training, workshops and meetings. With regular regional and interstate travel, you’ll contribute to continuous improvement, risk management and the delivery of person-centred care across the community.

What We’re Looking For:

  • Demonstrated experience in preparation and analysis of reports, quality compliance, accreditation, collection and maintenance of required data.
  • Demonstrated experience and proficiency in a Managerial /Supervising role and the supervision of staff.
  • Experience in the provision of aged care, palliative care, rehabilitation, and disability or community health services.
  • Experience in working in a community setting and undertaking community consultation and community development activities.
  • Experience in working within a multidisciplinary team.
  • Experience in Word, Excel, Email.
