Join our Mental Health Services Team as an Advanced Clinician - Psychosocial Transitions

 

The Advanced Clinician Psychosocial Transitions and Long Length-of-Stay is responsible for providing expert clinical, case coordination and consultative services in the care of patients presenting with complex psychosocial, physical, sensory, intellectual and/or multiple disabilities impacting on transition out of Mental Health Services.  

About the role: 

The role will assist the Allied Health Principal Leads, Mental Health Services, in providing effective leadership, governance and strategic direction to SALHN Mental Health Services and will promote multi-disciplinary core competencies. Duties for the role will include:

  • Provide leadership and expert specialist consultancy and advice/support to other health professionals in the broader treating team to support efficient and timely discharge planning processes for patients with complex psychosocial barriers to discharge
  • Develop and maintain positive working relationships with key external stakeholders such as the NDIA, DHW, ACAT, RACF providers, and Housing and Homelessness Services to advocate for systems/process change to support the discharge planning of complex inpatients.
  • Monitor and evaluate the efficiency of systems and processes for the management of complex and long length-of-stay patients.
  • Maintain and lead a complex direct clinical caseload of patients with psychosocial barriers to discharge.

About you:

  • Demonstrated high level experience in the provision of complex Social Work or Occupational Therapy assessment and intervention.
  • Ability to apply high level professional judgement to critically appraise complex clinical situations, consult with stakeholders and provide specialist advice and support to management and staff.
  • Ability to build and maintain positive and respectful working relationships with clinicians working directly with consumers and with other internal and external stakeholders 
  • Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards. (Mandatory for all clinical positions.)
  • Current contemporary and specialist knowledge of NDIA and NDIS, Aged Care, and Homelessness systems and processes for discharge planning complex inpatients.
  • Knowledge and understanding of the needs of vulnerable client groups including those with psychosocial disabilities and comorbid medical and other health needs.
