About the Role

In this rewarding role, you’ll provide compassionate and practical support to patients and their families as they navigate aged care placement and transition back to safe community living. Working closely with the Transitional Care Program (TCP) Service Manager, you’ll play a key role in helping individuals access the services, benefits and resources they need to ensure a smooth hospital discharge and future care planning.

As a vital member of the Placement Services team, you’ll build and maintain strong relationships with aged care facilities, health professionals, patients and families to ensure the best possible outcomes. Your ability to coordinate care, broker services and offer informed guidance will make a meaningful difference in people’s lives at a time when they need it most.

About You

You’re a compassionate and motivated professional who is committed to supporting patients and their families through the aged care placement process. You take pride in helping people access the right services to ensure safe and supported living arrangements.

  • You have strong knowledge of aged care and community care services, with the ability to connect patients to appropriate supports.

  • You demonstrate excellent communication and negotiation skills, fostering positive relationships with patients, families and service providers.

  • You are organised and detail-focused, able to maintain accurate documentation and coordinate multiple stakeholders effectively.

  • You bring empathy, professionalism and a genuine commitment to improving outcomes for older people and their carers.
