Join our Rehabilitation, Aged and Palliative Care Team as a Clinical Nurse working as part of the Fragility Fracture Prevention Team

This role will focus on supporting the Fragility Fracture Service (FFS) by identifying patients and utilising treatment pathways for the management of patients with, or for potential of, fragility fractures.

About the role: 

The role will work under alongside the FFS Nurse Practitioners (NP) and will utilise treatment pathways for the management of patients with, or for potential of, fragility fractures. The positions roles and responsibilities extend across the screening of patients, performing phone call follow-up and education, using clinical knowledge to escalate patient needs to the NP role and to enter clinical and patient data into established FFS data bases.  

The FFS provides comprehensive bone health assessments; health education on re-fracture risk; appropriate tests and investigations; referrals for treatment and multidisciplinary follow up / support.

About you:

  • Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience.
  • Demonstrated competence in rehabilitation nursing practice in accordance with the relevant standards
  • Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses.
  • Ability to prioritise workload and meet set timelines, whilst working under minimal supervision.
