Join our Rehabilitation, Aged and Palliative Care Team

The Social Worker participates as a member of the Virtual Rehabilitation Ward team, which provides care to older people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission. 

About the role: 

The Social Worker is responsible to the Director of the Virtual Rehabilitation Ward for providing Social Work services to inpatients and outpatients.

Duties for the role will include:

  • Contribute to safe and timely discharge and improved independence and occupational outcomes for clients
  • Undertaking home visits to assess requirements for the home environment.
  • Referring to and liaising with relevant community resources and services.
  • Liaising with the Senior Social Work and the Director, Social Work RAP about the provision of services and resources in the occupant’s work area.

About you:

  • Experience in the provision of clinical Social Work services to clients with a range of illness and disabilities relevant to older people and rehabilitation.
  • Ability to communicate with clients and a range of professional and other staff, orally and in writing, including in the provision of supervision to Allied Health Assistants and volunteers as required.
  • A degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
  • Organisational skills including the ability to prioritise work, meet deadlines, delegate appropriately and demonstrate initiative.
