About the Role

As a valued member of our interdisciplinary palliative care team, you’ll play a vital role in supporting patients and families through some of life’s most significant moments. You’ll provide compassionate counselling and bereavement support to individuals with complex psychosocial needs, helping them navigate their experiences with dignity and care. Your work will make a meaningful difference each day, offering emotional strength, guidance and understanding to those in need.

You’ll also contribute to the planning, delivery and evaluation of services that meet the unique psychosocial needs of patients and their families. Drawing on your expertise, you’ll provide specialist advice, consultation and supervision to colleagues, management and external agencies, helping shape high-quality, person-centred care across the organisation. About You

You’re a compassionate and experienced Social Worker who brings both professional expertise and genuine empathy to your work. You thrive in a collaborative team environment, are adaptable to change, and are deeply committed to supporting patients and families with complex psychosocial needs during palliative care.

  • Hold a recognised Social Work qualification and eligibility for AASW membership

  • Skilled in counselling, complex psychosocial assessment and family support

  • Experienced in providing psychosocial services within a health or palliative care setting

  • Knowledgeable in grief, loss and culturally responsive care for diverse communities
