About the Role

As a Clinical Nurse (CN) within the Multidisciplinary CALHN Integrated Care Geriatrics Service, you will play a vital role in providing expert nursing support, clinical assessment, and coordinated care to older community members with complex health needs. Your work will primarily focus on the Multidisciplinary Community Geriatrics Service (MCGS)—a dynamic, client-centred program dedicated to supporting older people experiencing cognitive or functional decline, medical or social complexity, and the challenges of maintaining independence at home.

Working collaboratively with a range of health professionals and community partners, you will deliver comprehensive assessment, triage, care coordination, and direct clinical intervention aimed at optimising each client’s wellbeing. This position offers the opportunity to make a tangible impact in improving quality of life and fostering independence for older adults, while being part of a supportive multidisciplinary team committed to holistic, integrated, and compassionate care.

About You

You are an experienced and compassionate Registered Nurse with a passion for supporting older adults through person-centred, evidence-based care. Your clinical expertise, strong communication skills and commitment to continuous improvement enable you to thrive in dynamic, multidisciplinary settings. You value collaboration, adaptability, and the opportunity to make a meaningful difference in the lives of vulnerable community members.

In this role, you will bring:

  • Clinical Excellence & Experience
    Registered Nurse with at least three years’ post-registration experience, demonstrating competence in nursing practice and experience working with older people across acute, community or residential care settings.

  • Leadership & Team Collaboration
    Proven ability to motivate, mentor and guide staff and students, fostering teamwork and maintaining high standards of care within multidisciplinary environments.

  • Communication & Problem-Solving Skills
    Strong written and verbal communication, conflict resolution and negotiation skills, with the ability to assess complex situations, formulate effective solutions and manage workload demands independently.

  • Knowledge & Adaptability
    Sound understanding of aged care systems, geriatric syndromes, quality improvement frameworks, and relevant legislation—paired with creativity and flexibility to meet evolving healthcare needs.
