Join Our Team as a Lifestyle Program Support Worker in Port Pirie – Make a Difference Every Day!

Are you passionate about enhancing the lives of older Australians and people with disabilities? Do you want to be part of a dedicated team that supports independence and social connection? We are seeking a compassionate and reliable Lifestyle Program Support Worker to join our team in Port Pirie.

In this fulfilling role, you will deliver centre-based activities for clients across various programs, including the Commonwealth Home Support Program, Home Care Packages, and NDIS. Your focus will be on providing social support, promoting independence, and creating a positive and inclusive environment for our Aged Day Program participants.  Key responsibilities:

  • Plan and deliver engaging activities for participants in line with relevant program guidelines.
  • Provide social support that promotes independence and well-being.
  • Support clients with transportation to and from day programs.
  • Assist in the development of personalised Support Plans and leisure assessments.
  • Ensure culturally appropriate service delivery and maintain accurate client records.

Essential Criteria

  • Completed AQF Certificate in Aged Care or a relevant qualification.
  • Experience in delivering direct care services, particularly to aged, frail, or disabled individuals.
  • Strong administrative skills, including proficiency with Microsoft Office.
  • Ability to work collaboratively within a team.
  • Compassionate and dedicated approach to client care.
