About the Role:

We have an exciting opportunity for a Diabetes Education Service - Nurse Consultant to join our dedicated team within Northern Adelaide Local Health Network. You will provide advanced clinical leadership, education, and consultancy in the management of diabetes across acute and primary care settings.

As an expert practitioner, you will play a pivotal role in supporting both patients and clinicians to achieve optimal outcomes through evidence-based practice, innovation, and service improvement.

You will work collaboratively within a multidisciplinary team to coordinate complex care, deliver structured diabetes education, and promote self-management strategies that empower individuals and families affected by diabetes.

Key Responsibilities

  • Provide expert clinical assessment, management, and consultation for people with complex diabetes needs.
  • Lead and deliver structured diabetes education outpatient clinics.
  • Facilitate professional education and mentoring for nursing and allied health staff to build workforce capability in diabetes management.
  • Lead and participate in quality improvement, clinical audit, and research activities to enhance service delivery and patient outcomes.
  • Contribute to policy development, clinical governance, and strategic planning for diabetes services within the NALHN.
  • Foster collaborative partnerships across hospital, community, and primary care settings to improve continuity of care.
