Join Our Team as a Direct Care Worker (Community) – Support Independence and Wellbeing

Make a meaningful difference by helping aged, frail, or disabled clients remain living safely and comfortably in their own homes. This is your opportunity to deliver person-centred care, support independence, and work collaboratively within a multidisciplinary team.

About the Role:

As a Direct Care Worker (Community), you will assist clients with a range of daily living activities, identify and respond to individual needs, and provide diversional therapy programs for individuals and groups. You will collaborate closely with Community Health professionals and liaise effectively with nursing, maintenance, and other staff to ensure a safe and supportive environment.

Your responsibilities also include contributing to the efficient operation of the service through program setup, operation of relevant equipment, and maintaining accurate records. You will provide guidance and support within your area of expertise, participate in training, and maintain your knowledge and skills to ensure high-quality care. Employees may be required to perform duties at lower levels as needed.

What We’re Looking For:

  • Completed AQF Certificate III relevant to the position and at least 560 hours satisfactory in-service experience.
  • Completed at least 560 hours satisfactory in service experience.
  • Proven experience in exercising own judgment and initiative in the day to day execution of a position.
  • Demonstrated experience in the provision of a direct care service in a health-related field.
  • Demonstrated experience in dealing with aged, frail or disabled persons.
  • Experience in the use of computer packages e.g. Microsoft Word, Excel.

  • Immunisation Risk for this position is - Category A
