Empower Community Wellbeing as a Day Program Support Officer with YNLHN!

Are you passionate about supporting older people to stay connected, engaged, and independent? Join YNLHN as a Day Program Support Officer and play an important role in delivering meaningful centre-based activities for clients across CHSP, Home Care Packages, NDIS, and other programs. This is your chance to make a positive difference in the wellbeing and daily lives of older community members.

Key Responsibilities:

  • Deliver social support and diversional therapy activities in line with program guidelines.
  • Plan and run engaging centre-based programs with the Day Program Coordinator.Promote independence, social participation, and client choice.
  • Respond to enquiries, provide courteous support, and maintain culturally sensitive practice.
  • Transport clients to and from programs, including driving the Day Program bus.
  • Assist with support plans, assessments, documentation, and accurate record keeping.
  • Maintain effective teamwork, communication, and participation in program development.

What We’re Looking For:

We’re seeking a supportive, enthusiastic individual who enjoys working with older people and can build positive relationships with clients, families, and colleagues. You’ll bring strong communication skills, the ability to follow procedures, and confidence in delivering group activities. A team-focused mindset, cultural sensitivity, and readiness to learn and develop within the role are essential. A completed AQF Certificate relevant to Aged Care is required. Your commitment to quality care and meaningful engagement will help clients thrive.
