About the Role:

As a Clinical Nurse – Geriatrics in the Home, you will:

  • Deliver clinical nursing care to geriatric patients in home settings
  • Conduct comprehensive health assessments and develop individualised care plans
  • Coordinate with multidisciplinary teams to ensure seamless patient care
  • Provide education and support to patients and their families
  • Maintain accurate clinical documentation and reporting

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate, and have at least 3 years (full-time equivalent) post-registration experience.

You will have strong clinical skills in aged care and community health settings, and a passion for delivering high-quality care to older adults in the comfort of their own homes.

In this role you will have strong interpersonal and communication skills which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships. An ability to effectively manage multiple, diverse and competing priorities and demonstrated resilience within a stressful and complex environment is essential to this role.

As a community based role, you will be required to have a valid drivers licence.

Our Local Health Network Sites and Services:

Lyell McEwin Hospital The major hospital in the north and north-east for emergency care, complex and multi-day surgery, medicine, obstetrics, paediatrics and outpatient services.

Modbury Hospital A general hospital with emergency care, elective surgery, medicine, outpatient and sub-acute services, rehabilitation, geriatric and palliative care.

Mental Health - The Division of Mental Health provides a comprehensive range of public mental health services for youth, adults and older people in northern Adelaide. Services are provided through community health centres and hospitals, and to consumers in their own homes. 

Forensic Mental Health - A statewide service that operates as part of the Division of Mental Health for forensic patients and prisoners. 

Community Sites - Elizabeth GP Plus, Modbury GP Plus and Gilles Plains GP Plus Super Clinics providing community based care.
